each designed to specifically meet the needs of patients with similar
diseases and conditions. These specialized programs streamline the
involved assessments and clinical pathways for achieving desired
patient outcomes, whether they are safety, improved mobility or
increased patient independence.

Orthopedic
Total Joint Program
Patients undergoing scheduled orthopedic surgery often want to know
who will be taking care of them while they recover in their homes.
Home Health Services' complimentary pre-surgical home visit introduces
orthopedic patients to what to expect after their surgery and assists
in alleviating possible anxiety over their recovery. During the
patient's post-operative hospital stay, a Home Health Services onsite
case manager visits the patient and family again to coordinate for
the eventual discharge from the hospital into home care.
Cardiac
Education Program
Patients hospitalized for cardiac conditions or cardiac surgery
often experience anxiety and despair over how their lives will change
after they leave the hospital. They fear that they will not be able
to resume their normal activities and often feel that their best
years are behind them. Home Health Services' Cardiac Education Program
helps cardiac patients and their families understand how to best
manage their condition and live a full life under medical recommendations.
The HHS team addresses multiple guidelines for everyday living,
including lifting, driving, maintaining an appropriate diet, walking,
smoking cessation and coping with lifestyle changes.

Parkinson's
Home Safety Program
Home Health Services' Parkinson's Home Safety Program provides patients
and their caregivers with a better understanding of the Parkinson's
disease process, associated moods and behavior changes. The program's
inclusion of caregivers in therapy and education on community resources
empowers them to deal with the disease and helps curb many potential
risks for caregiver burn out. The program's tips and reminders on
everyday living, such as bathing, feeding and using appropriate
adaptive devices, provides the patient and caregiver with the knowledge
and tools for managing the patient's independence at home.
Dementia
Home Safety Program
Imagine not knowing where the bathroom is in your home, or not remembering
how to hold a spoonor even more devastating--not recognizing
your loved ones. Home Health Services' Dementia Home Safety Program
provides patients and their caregivers with an understanding of
their disease process, associated moods and behavior changes. It
helps patients limit their own confusion and teaches them about
the best methods to deal with the disease's issues.

Mastectomy
Program
Patients undergoing scheduled mastectomy surgery can find reassurance
in meeting with their home care provider prior to surgery to know
who will be caring for them while they recover at home. Home Health
Services' complimentary pre-surgical home visit introduces mastectomy
patients to what to expect after surgery and assists in alleviating
possible anxiety over their recovery.
A Home Health Services onsite case manager will visit the patient's
home prior to surgery to assess psycho-social needs, answer questions
and allay anxiety over the surgery. In addition, during the mastectomy
pre-op visit, the onsite case manager can perform pre-operative
teaching on care of the incision site after surgery.
Back/Spine
Program
Patients undergoing scheduled back or spine surgery often want to
know who will be taking care of them while they recover in their
homes. Home Health Services' complimentary pre-surgical home visit
introduces patients to what to expect after surgery and assists
in alleviating possible anxiety over their recovery. During the
patient's post-operative hospital stay, a Home Health Services onsite
case manager visits the patient and family to coordinate for the

Pulmonary
Self-Management
Home Health Services' Pulmonary Self-Management Program assists
patients with managing their respiratory condition or disease and
identifying contributing factors. A respiratory therapist, specialized
in working with respiratory patients and rehabilitative teaching,
leads the program. The respiratory therapist will assess the home
environment for external triggers to the patient's disease, such
as improper kitchen ventilation and overuse of aerosols. She will
also teach the patient to monitor for potential problems, such as
watching for high pollen counts through the news and discussing
realistic goals for smoking cessation.
Bridge
to Hospice
The Bridge to Hospice Program focuses on educating patients on the
principles and philosophies behind hospice, while still providing
skilled needs at home, whether they are for nursing, physical or
speech therapies. Medicare requires at least one of these skilled
needs for home care, but home health aides, other therapies and
social work consultations supplement these core visits to create
a program that prepares the patient for the benefits and services
of hospice.
Diabetes
Education Program
The Diabetes Education Program teaches newly diagnosed patients
about their disease, their conditions and the lifestyle changes
that will be required. The home care team can supervise physical
activity programs and provide emotional and social support, empowering
patients with a sense of control over their future lives and health.

Nursing:
Home Health Services has a full staff of registered nurses (RNs)
and licensed vocational nurses (LVNs). An RN normally does each
admitting patient's evaluation and assessment. Nurses also supply
ongoing skilled needs such as wound care, medication administration
and teaching.
Occupational
therapy: The occupational therapist provides assistance
in household management skills, energy conservation and work simplification.
This therapist uses methods that attempt to improve a patient's
cognitive and functional ability to perform basic activities of
daily living.
Physical
therapy: The physical therapist provides goal-directed approaches
to rehabilitation of a patient with decreased ambulation, gait difficulties,
weakness and balance disturbances.
Speech
therapy: The speech therapist can provide assessment and
evaluation of neurological and cognitive disorders that cause difficulty
with communication.
Social
work: In addition to nursing and therapies, a Home Health
Services social worker can perform visits for patients requiring
assessment and possible referrals to community resources for social,
emotional, financial and personal problems related to their illness
and recovery.
Home
health aide assistance: The home health aide supplements
a patient's basic nursing or therapy care by providing assistance
with personal care and hygiene needs.

Home Health Services provides Dallas/Fort Worth businesses with
onsite flu shot clinics during October
and November each year. Home Health Services' nurses specialize
in providing year-round health care outside of
the hospital and doctor's office. During flu season, they concentrate
on setting up flu clinics to help the community prevent unnecessary
trips to the doctor brought on by the flu. For more information,

Home Health Services provides thousands of dollars in free or sliding
scale home care each year for members of the community without medical
coverage.
Bridge
Medication Assistance Program
The Bridge Medication Assistance Program provides eligible Home
Health Services patients with short-term financial assistance while
they transition home after a hospital stay. This bridge program
is designed to allow eligible Medicare patients time to locate community
resources and apply for available drug manufacturers' indigent programs.
Ryan
White Program
Since 1996, Home Health Services has provided skilled health care
services in the homes of patients living with AIDS.
